STUDENT FORMS

These forms are designated for students:

Form Description
Assignment Deadline Extension Appeal If you have an extenuating circumstance that will prevent you from turning in your work on-time, you may fill out this form and ask for an extension. (Note: 2 weeks is the maximum extension.)
CampusVue Student Info Correction Form If your student information is incorrect on CampusVue, fill out this form to let the SUM Administration know what you would like changed.
Community Covenant Form (Returning Students) All students must fill out this form after being accepted to SUM.
Course Add / Drop / Switch Form (FALL) The last day to add or drop Fall 2018-19 classes is Sep 2, 2018. The last day to withdraw classes is Oct 19, 2018.
Course Add / Drop / Switch Form (WINTER) The last day to add or drop Winter 2018-19 classes is Dec 2, 2018. The last day to withdraw classes is Feb 1, 2019.
Course Add / Drop / Switch Form (SPRING) The last day to add or drop Spring 2017-18 classes is April 6, 2018. The last day to withdraw classes is May 18, 2018.
Degree Program Change Form Use this form to laterally change your degree program. View the Undergraduate Degree or Graduate Degree page for more information.
Directed Research Add/Drop Form Fill out this form to add or drop Directed Research courses (independent studies) only.
Enrollment Agreement Form Incoming students must fill out this form to complete the application process.
Enrollment Verification Request Use this form to prove that you are a current SUM student.
FERPA Release Form The Family Educational Right and Privacy Act gives parents certain rights with respect to their children's education records. All SUM students must sign this form.
Financial Aid SAP Appeal Form Students may appeal to continue receiving financial aid if their eligibility for financial aid has been revoked due to unsatisfactory Student Academic Progress (SAP). Please review the SAP Appeal Guidelines and Procedures before submitting.
Grievance Form Students may initiate a conflict resolution by filling this form.
Intent to Graduate Form Notify the Academic Department of your intent to graduate and attend a graduation ceremony. Note: Failure to fill out may prevent graduation.
Internship Agreement Form Senior students must find a church leader or pastor to supervise their internship. Both the student and supervisor must fill out this form.
Leave of Absence Form Fill out this form if an extenuating circumstance will preent you from attending two or more weeks of class, but you intend on returning to complete the trimester.
Location Transfer Form Students and Cohort Advisors may fill this out to transfer the student from one SUM cohort to another SUM cohort.
Mardi Gras - Leave of Absence If you are an enrolled SUM student who cannot make it to Mardi Gras 2018, please fill out this form with documentation, so that it can be reviewed and approved by the Dean of Student Ministry.
Non-Degree Enrollment Agreement Fill out this form to be approved to take classes prior to receiving your High School Diploma or GED.
Password Reset Form If you have forgotten your CampusVue, Gmail or SUMoodle password, fill out this form to have it reset.
Practicum Site Agreement Form Students may use this form for the Practicum Site. Instructions are located here.
Records Request Form Students may use this form to request disciplinary records from the Academic Department.
SOA Evaluation Form Every term, students should complete this form. Be sure to write in your Cohort Advisors's name and email so that he/she can complete the second portion of the evaluation.
Student ID Form Newly accepted students will need to submit this form within the first few weeks of the term.
Transcript Request Form Fill out this form to have SUM send a sealed, official transcript to you or an outside institution.

FACULTY FORMS

These forms are designated for faculty members:

Form Description
Academic Dishonesty Report Fill out this form if a student is found to be cheating or plagiarizing.
Faculty Feedback Form Send anonymous feedback for SUM classes, students, cohorts, etc.
Grade Change Form If there is an incorrect grade in CampusVue, professors and Cohort Advisors should correct the grades in Canvas, and let SUM know about the changes using this password-protected form.
Grievance Form Faculty may initiate a conflict resolution by filling this form.
Inactive Student Attendance Form Notify SUM if a student has missed two consecutive weeks of class.
Password Reset Form If you have forgotten your CampusVue, Gmail or SUMoodle password, fill out this form to have it reset with a temporary password.
Reimbursement Request Form Use this form to request a reimbursement.

COHORT ADVISOR FORMS

These forms are designated for Cohort Advisors:

Form Description
Administrative Withdrawal Form Fill out this form if a student has stopped attending classes for over two weeks, and you are unable to reach them.
Cohort Dissolution Form When a Partnering Ministry is no longer interested in maintaining the partnership, this Cohort Dissolution Form must be completed by the Visionary Leader and the Cohort Advisor.
SOA Accountability Committee Recommendation Advisors should fill this out any time a student has Student Life issues that need to be addressed. This document will go before the Director of Student Life and Ministry and the Executive Committee. Read the instructions first to learn how to use the form here.
SOA Incident Report SOA Incident Report is a form to fill out if your student has committed a minor infraction that does not constitute potential suspension from SUM. Ex. Tardiness, Dorm Issues, Bad Attitudes, etc. This form should be kept in your records. If the student's behaviors ever escalate to the SOA Accountability Committee these documents will be used.
Email Password Reset Form If you have forgotten your SUM email password, fill out this form to have it reset with a temporary password.
Grievance Form Cohort Advisors may initiate a conflict resolution by filling this form.
Location Transfer Form If a student intends on relocating to a different campus or cohort, please notify an SUM Administrator by click this Link.
