Students may appeal to continue receiving financial aid if their eligibility for financial aid has been revoked due to unsatisfactory Student Academic Progress (SAP). Please review the SAP Appeal Guidelines and Procedures before submitting.
Advisors should fill this out any time a student has Student Life issues that need to be addressed. This document will go before the Director of Student Life and Ministry and the Executive Committee. Read the instructions first to learn how to use the form here.
SOA Incident Report is a form to fill out if your student has committed a minor infraction that does not constitute potential suspension from SUM. Ex. Tardiness, Dorm Issues, Bad Attitudes, etc. This form should be kept in your records. If the student’s behaviors ever escalate to the SOA Accountability Committee these documents will be used.
